-We provide breakfast, lunch and afternoon snack for all the children in our care. We do require all our families to participate in our meal program.
Medical Plan of Care
-This form should be completed for any food allergies or sensitivities or for any asthma or other severe illness or accommodation.
Income Form (CACFP)
-Needs updated annually for our meal program
Enrollment Paperwork
Please contact the center to schedule a virtual tour prior to starting the enrollment process. We will do a 30 minute virtual tour, 15 minute walk through tour and then our enrollment process.
We will NOT do an enrollment without a virtual tour!
Child Health Assessment Form
-Should be completed by the physician at EVERY Well-Child Appointment along with vaccination records

-Needs updated every 6 months
Parent Fee Agreement
-Needs updated every 6 months
Emergency Plan Letter and Release
